On December 3, 2018 at 5:13 PM, officers responded to the area of 1408 West Broad Street in
reference to a motor vehicle accident involving a motorcycle. Upon officers’ arrival, they
discovered 32-year-old motorcyclist Phillip Frady of Bloomington Springs deceased from the
crash.
Initial investigation indicated that Mr. Frady was operating a 1985 Honda Motorcycle while
traveling westbound on West Broad Street. A dark colored pickup truck, unknown driver, was
traveling eastbound on West Broad Street attempting a left turn onto Broadway Avenue. The
driver of the truck turned into the path of Mr. Frady causing the fatal accident. The truck left the
scene prior to police arrival.
The Cookeville Police Department is seeking the public’s assistance in learning the identity of the
driver of the truck. Officers are currently working several leads in attempts to identify the driver.
This crash remains under investigation. We encourage anyone who may have witnessed the
